Each one is kept with the session it belongs to, and a treatment that requires consent is stopped from starting until it is in the record. A professional can override, with a reason that gets recorded.
Photographs, with permission
Photo capture and publication permissions are recorded per customer, with who recorded each one.
Notes that are signed
Dated, and signed by whoever wrote them. Nothing with history is deleted: a customer is archived, a duplicate is merged without losing either history.
