Consents and forms
Record your customers’ signed authorizations and fill in your business’s questionnaires on their record.
Reviewed on Saturday, October 24, 2026
MAYEL handles three separate authorizations: Clinical consent (to receive treatments), Photo capture (to take clinical photos) and Photo publication (to publish them). Granting one never implies another.
Record a consent
1In the record, Documents tab, Consents card, tap Record a consent.
2Choose the Authorization and enter the Document version (for example, “v2 2026”).
3In Signed by, enter the name exactly as it appears on the signature: the customer or their guardian.
4If it’s for a specific treatment, choose it in Treatment (optional).
5Attach the Signed documents (optional): a PDF or photos of the pages.
6Tap Record a consent.
If the customer withdraws their authorization, tap Revoke on the In force consent. From then on MAYEL blocks whatever depended on it, and the original signature is kept in the history.
Forms
Forms are your business’s own questionnaires: an intake form, a post-session check. You create them in Settings → Customers → Forms with Add a form. To fill one in, in the Documents tab tap Fill in a form, choose which one, answer it and tap Save.
Do I have to ask for consent at every appointment?
No. Clinical consent is checked per customer, not per treatment: if they already have one in force, they don’t sign again each time.
I changed a form. What happens to the ones already filled in?
They still read the same: the questions are saved exactly as they were when the form was filled in.
